This Medical Membership Program (“Membership Program”) is offered by NorthStar (“we,” “our,” or “us”) to provide members with access to certain healthcare-related services, wellness support, and membership benefits as described in the applicable membership agreement.
By enrolling in or participating in this Membership Program, you acknowledge and agree to the following terms and disclaimers.
1. Membership Is Not Insurance
The Membership Program is not health insurance and is not intended to replace health insurance coverage. Membership fees do not cover hospital services, specialist care, emergency services, surgeries, laboratory testing, imaging, prescription medications, or other healthcare expenses unless specifically stated in writing. Members are strongly encouraged to maintain active health insurance coverage for services not included in the Membership Program.
2. No Guarantee of Outcomes
Participation in the Membership Program does not guarantee specific medical results, health outcomes, or improvements in condition. Healthcare outcomes vary by individual and depend on many factors, including medical history, adherence to treatment plans, and lifestyle factors.
3. Scope of Services
